Meet the Team
While the country’s first Transitional Healthcare team was established in September 2019, Christine and Maranda’s journey started in 2015 at the Indiana Department of Correction (IDOC). Christine was working for IDOC’s contracted medical vendor helping parolees transition into the community and Maranda, who had been working at IDOC since 2006, was working in case management. They quickly identified that a missing piece of an incarcerated person's reentry journey was their link to physical and behavioral healthcare. Their mission became boosting transition planning for releasing persons by wrapping them in healthcare services that addressed social determinants of health.Together they worked with community partners to build out a Transitional Healthcare plan that kept the community safe and prevented justice responses to a community care issues. Their continued work together and success with difficult medical releases lead to the official establishment of a Transitional Healthcare team in 2019 within the Health Services Division of IDOC.
